Should Young Children Wear Masks?

There’s no one easy answer, but experts agree that the main reason to give a child a mask is to avoid infecting others.

Maybe my squirmy preschooler would be willing to wear a mask if it featured Disney characters?

I really should have known better.

Nope, my daughter said. No mask.

“Even if it’s a ‘Frozen’ mask?” I asked. “With pictures of Anna and Elsa?”

My daughter wagged her little finger. “No, I’m not wearing any mask outside,” she replied. Then (in case I still wasn’t getting it) she added: “If I go outside with a mask, I’m just gonna take it off my ears.”

The C.D.C. has said that cloth face coverings should not be placed on children under 2, nor should they be used on anyone who has trouble breathing — but other than that, the agency has offered no advice about how young children should be using them. Further confusing matters, the advice to wear a mask is not a requirement, and President Trump has already said that he probably won’t follow the guidelines.

The C.D.C. suggests using face coverings in public where it’s difficult to stay more than six feet apart from other people, and especially in areas where the virus is spreading rapidly, like in New York City. So, is it crucial for my preschooler to wear one? And if so, could I convince her to wear it safely and properly?

To find out more, I spoke with pediatricians who specialize in infectious diseases. As I discovered, for parents of children who are 6 or under, much of what you decide will depend on where you live, your child’s personality and the places you frequent.

On April 6, the C.D.C. published preliminary findings on pediatric coronavirus cases in the United States. According to the report, 2,572 cases occurred in children younger than 18, and those children were significantly less likely to become seriously ill from the virus than American adults were. They also appeared less likely than adults to develop the main coronavirus symptoms like fever, cough or shortness of breath.

These findings further bolstered the idea that children who become infected might have mild or undetected cases of the disease and could spread the virus to others in their families and communities.

Using a cloth mask on your children can help prevent them from infecting others. If they have the virus, a face covering can prevent their respiratory droplets — the little virus-laden particles that are expelled from mouths or noses while talking, coughing or sneezing — from landing on or around other people, said Dr. Mark Sawyer, M.D., a pediatric infectious disease specialist at the University of California San Diego School of Medicine and Rady Children’s Hospital-San Diego.

Nonmedical face coverings won’t offer complete protection to you or your child. But if used properly, they are better than nothing.

Although children have made up a small proportion of coronavirus cases so far, some have become very sick.

The C.D.C. has estimated that as many as 25 percent of people with coronavirus may not show any symptoms, which is one reason the virus has spread so rapidly.

Masks are most useful in public places where your child is likely to come within six feet of another person (for example in a grocery store or pharmacy) and in areas where the virus has been spreading quickly, the C.D.C. said.

If you’re in an environment where you can easily stay at least six feet from other people, like an empty soccer field or a sparsely populated hiking trail, you and your children do not need a mask.

Your child also does not need to wear a mask at home. If someone in your household is ill, try to isolate the sick person and have your child stay at least six feet away if possible, the experts said.

If your child is sick or you think she might be sick, try to avoid going out in public if you can, Dr. Sawyer advised.

But if you need to take your child to the doctor’s office, call ahead of time to let them know you’re coming and wear a mask if you have one.

This is far trickier. Some children will be more open to wearing a face covering than others.

“For anyone who has a toddler, it’s going to be a challenge to keep it on,” Dr. Sawyer said. But, he said, having them at least try to wear one is better than having them not try at all.

He advised first explaining to your child why other people are wearing masks and why it is important — because people can spread the virus before they’re showing symptoms, for instance. Set an example by wearing a mask yourself and be sure to offer lots of positive reinforcement when your child does wear one, he added.

You may also want to consider a reward system, suggested Dr. Meg Fisher, M.D., a pediatric infectious disease specialist and the medical director of the Unterberg Children’s Hospital at Monmouth Medical Center in Long Branch, N.J.

For example: If your child wears the mask, you could praise her efforts with a healthy treat like strawberries or yogurt, or let her pick out a fun sticker.

That being said, parents of toddlers should avoid pushing too hard.

“Forcing a child to do something they don’t want to do in the 2- or 3-year age range is asking for trouble,” Dr. Fisher said. “That’s an age where tantrums are common, and the last thing we want is a child to have a tantrum and put themselves in a position where they’re either running away from a parent or running into danger.”

Younger children — particularly those in the earlier developmental stages — will find it more difficult to understand the importance of a mask, Dr. Fisher said. “They will put their hands all over the mask, and put it on and off and bring it down below their nose and above their nose and therefore it won’t be doing anything that it’s supposed to be doing,” she said.

In the end, Dr. Fisher added, their age and maturity level will determine how likely they are to cooperate.

“It would be a special 2-year-old who would tolerate it,” she said. So parents should use their best judgment.

The most effective thing parents can do is teach their children to keep their fingers out of their face and practice hand washing, Dr. Sawyer said. But don’t expect that you’ll have an easy time altering their behavior.

Masks of any kind should never be used on children younger than 2 because of the risk of suffocation. But the C.D.C. has said that it is safe for children aged 2 and older who do not have breathing problems to wear a cloth face covering made of cotton.

If you are staying at least six feet away from others, the mask itself will not become contaminated. The purpose of the mask is to catch your droplets so that you don’t infect others, Dr. Fisher said.

You may also be wondering how often to wash your cloth masks. C.D.C. guidance says that masks “should be routinely washed depending on the frequency of use,” but offers little else in terms of specifics.

“There is really no information on this,” Dr. Fisher said. “Certainly masks should be washed when dirty. I think washing them weekly either by machine or by hand would be reasonable. Frequent washing may make the masks not fit as well.”

No. Everyone, including children, still must practice physical distancing, even if they are wearing a mask. Avoid gathering in groups and try to stay home unless you need to go outside for exercise, groceries or medicine.

Cloth masks are simply a physical barrier to keep droplets away — much like when you cough into your elbow — and do not offer the kind of protection afforded by medical masks like surgical masks or N95 respirators.

The C.D.C. is recommending cloth masks because surgical masks and N95 respirators are in short supply and should be reserved for health care workers and emergency medical workers.

You can buy a cloth mask online or make your own, at home with materials like a T-shirt or bandanna: The C.D.C. posted do-it-yourself instructions online, including a method that doesn’t involve sewing.

For children, a standard cloth mask is all that’s necessary, the experts said.

Some people are making masks with a pocket to insert a coffee filter, and the C.D.C. shows how to do this using a bandanna. But experts don’t recommend those for children because they are far less breathable.

“You really don’t need a coffee filter to stop the big droplets,” Dr. Fisher said. The more efficient masks like the N95 respirators “are to protect someone who’s going into a room with a person who has the disease, and they’re doing a procedure that’s going to make the virus go into the air, so you have a much higher risk — you’re in a completely different situation than if you’re just going outside walking around.”

Using a face covering with a filter could provide more safety if you’re an adult who is caring for a sick loved one. But if you’re making one of your own, be sure to use a coffee filter, like the C.D.C. recommends, and never a vacuum filter, because vacuum filters have fiberglass particles that should not be anywhere near one’s face, experts said.

 

 

Does Your Kid Have to Wear a Mask Outside?

The CDC is asking all adults to wear masks anytime they step out right now. But what about kids? Here are the guidelines.

 

Health officials have given us whiplash over the past several weeks with their guidance on masks. First, the Centers for Disease Control and Prevention (CDC) said only sick people should wear one. Now, they recommend that almost everyone over the age of two sport a cloth mask in public to limit transmission of COVID-19. Some areas, including Los Angeles and Miami, are even ordering residents to wear masks in public. But keeping a mask on your kid’s face is tricky, to say the least. Is the fight actually worth it?
“If you can get a child to keep a mask on their face when they are outside, not indoors, you should,” says Dyan Hes, the medical director of Gramercy Pediatrics in New York City and a pediatrics professor at Weill Medical College of Cornell University. Hes realizes that’s easier said than done. “They rip them off in like three seconds,” she says, but it’s important to try anyway.
In a set of guidelines released yesterday, the American Academy of Pediatrics offers a few caveats to the all-mask-all-the-time rule. They include skipping masks when kids are at home, taking special precautions with kids who have severe cognitive or respiratory impairments (including monitoring them with a pulse oximeter) and also, a bit confusingly, not worrying so much if people aren’t around and kids can keep their hands off oft-touched surfaces:

If children can be kept at least 6 feet away from others, and not be in contact with surfaces that could harbor the virus, then they do not need a mask for the protection of themselves or others.

For example, during a walk outdoors, as long as children can maintain social distancing of more than 6 feet and do not touch tables, water fountains, playground equipment or other things that infected people might have touched, then they will not acquire the infection and would not need masks.

If this sounds like an impossible ask for your kid (not touching surfaces is a high-bar indeed for elementary-aged and under), you probably should start training your kid on a mask when they’re outside.

The purpose of wearing a mask isn’t to keep your kid safe so much as to prevent your little germ incubator from spreading disease. As many as 25 percent of cases of COVID-19 are asymptomatic, the director of the CDC told NPR. Many of those who are infected without symptoms are probably kids. “All children should be wearing masks because they’re probably the ones who are shedding the virus.” Covering your child’s face helps prevent them from spraying droplets with coronavirus into the air, whether they seem sick or not.
Masks should have a snug fit against the side of the face. Medical-grade masks designed for kids’ small faces are difficult to find, and adult models probably won’t fit your kid. But you shouldn’t be using them anyway. The CDC recommends saving medical and surgical masks that are in short supply for healthcare workers. The general public should use cloth masks, which you can make out of materials like bandanas or T-shirts. Atrium Health provides a guide for sewing cloth masks to fit both adults and young children. But anything is better than nothing, Hes says. “You should have some kind of mask on, whether it’s a scarf, a bandana, or some other kind of protection.” She’s even heard of people wearing ski masks or eye masks over their mouth and nose.
Infants, however, get a pass with the mask, according to the CDC. Obstructing the airway of kids age 2 and younger is dangerous, Hes says. But age 2 is not a hard cutoff. If you think a mask is risky for your child, trust your gut. “You have to use judgment as a parent,” Hes says. If you must bring your infant into public, limit exposure by pushing them in a stroller with a sunshield or rain cover. In addition, children who are having trouble breathing or may be unable to remove their own mask should not wear one.
Wearing a mask alone, especially a cloth one, isn’t enough to stop the spread of COVID-19. Cloth masks don’t have the same antiviral properties as the medical kind. “They help a bit, but they’re not foolproof,” Hes says, so social distancing is just as important as ever. To make your child’s mask as effective as possible, have your kid wash their hands before they put their mask on and after they take it off, according to the World Health Organization. Clean the mask in the washing machine as soon as it gets damp, and remove it from the face by using the straps around the ears. And if your child touches their mask while wearing it, have them wash their hands or use hand sanitizer. If that sounds like a lot of hand-washing, that’s because it is. Don’t skimp, and stay safe.
